Endoscopic assessment is crucial to the treatment of biliopancreatic diseases. As of yet, the wall load during this type of endoscopy is unknown, as it is not directly measurable in patients.
Therefore, in this study an endoscopy training model comprising an esophagus, a stomach, and a duodenum, was redesigned and five areas with increased wall stress during endoscopy in the model were identified using different duodenoscopes. These areas were equipped with a sensor system consisting of five load cells. Fifteen standardized duodenoscopies were performed.
The wall stress during duodenoscopies was measured intraprocedurally for the first time in this work using a synthetic, human–analogue model.

Artificial urological model for endoscopy training

Current training of minimally invasive surgery often relies on animal organ models or physical organ phantoms, which do not offer realistic surgical scenes or feedback for surgeons to improve their skills. In this study, a high-fidelity artificial urological system is reported that allows realistic simulation of endoscopic urological procedures and offers a quantitative assessment of the surgical performance.
The physical organ model was fabricated by 3D printing and polymer molding based on human CT data. The system resembles the human upper urinary tract, with a high-resolution anatomical shape and sub-millimeter vascular patterns embedded on the inner walls of the renal collecting system. During surgical simulation, endoscopic videos are acquired and analysed to quantitatively evaluate performance skills by a computer algorithm. Endoscopic professionals and trainee groups were recruited to perform surgical procedures on the system, and significant differences between the two groups of surgeons have been determined.
The surgical simulator offers training of endoscopic urological procedures in a realistic and safe environment, and the quantitative evaluation of endoscopic skills.

The Freeform Reversible Embedding of Suspended Hydrogels (FRESH) 3D bioprinting approach was used to generate complex 3D tissue and organ models with high cell viability using a wide range of biocompatible materials. The FRESH technique enables complex tissue fabrication by using a support bath that holds the printed materials or bioinks in place until they are cured. Using this method, researchers have demonstrated that large-scale 3D bioprinting of soft tissues and other complex tissue scaffolds with properties comparable to human tissues and organs is possible. This work is opening new avenues for tissue engineering and has already produced several research advancements, including the printing of organ models from patient-specific medical images and the development of a beating cardiac tissue, which has a potential application in surgical training.

The FRESH method (Freeform Reversible Embedding of Suspended Hydrogels) offers the advantage to create embedded complex tissue scaffolds, which are quite soft, from hydrogels which feature a high similarity to native tissues and organs. Up to now size ranged between a few millimetres to a few centimetres, but now the range could be expanded to a full-size model of a human heart. MRI scans from a patient's heart were used to create the organ copy made of alginate. The model mimics the elasticity of real cardiac tissue, while it is stable enough to perform suturing. The method can be used in surgeons' and students' education as well as for pre-surgery planning.

Patients with congenital heart disease may be treated with percutaneous pulmonary valve implantation to treat narrowed or leaky pulmonary valves. However, commercial options are not always available for children or special subjects. Over the years, the handmade pulmonary valved conduit has provided a strategy to customize the size to the specifications of the patient. In the present study, the researchers aimed at improving the design by generating a meta-learning-based intelligent model to train the physician to determine optimal parameters for customized valve reconstruction. This model can overcome problems arising from empirical parameter determination. The customized handmade pulmonary valved conduit produced thanks to the model were validated in vitro by assessing the regurgitation fraction and the heart pump efficiency using a circulation loop system.

Virtual training platform for cardiovascular surgery

Training in cardiovascular interventional surgery (CIS) has been mainly performed under fluoroscopic guidance in animals. However, this has drawbacks due to the anatomical differences between animal models and the human body.
In this study, the researchers present a virtual training platform for cardiovascular interventional surgery (CIS) that creates a realistic simulated environment for training basic techniques.
The platform consists of a mechanical manipulation unit, a simulation platform, and a user interface. A decoupled haptic device provides dynamic behaviour and high-quality force feedback. In addition, an efficient physics-based hybrid model is used, which can realistically and interactively simulate the complex behaviour of guidewires and catheters. Three simulation studies show that the platform has reasonable accuracy and robustness. This platform enables realistic simulation of vascular surgery technique testing such as guidewire and catheter simulation and stent placement, as well as rendering techniques.

Digital model of epicardial pacing for training of physicians

Improvement of postoperative management of patients having cardiac surgery could be obtained with increased understanding of epicardial pacing, In the present study, the researchers aimed at developing a software-based epicardial pacing program to be used with the existing patient simulator. These digital training tools should assist junior physicians to become competent in the management of epicardial pacemakers. The tools developed were based on a human patient simulator and Flash animation to present arrhythmias and epicardial pacing interventions. The trainee is able to make adjustments (type of pacing, rate of pacing, and chamber output) as if using an actual pulse generator. The study reports the training of 15 anesthesiology residents who had minimal epicardial pacing experience. An anonymous post-scenario questionnaire was used to receive feedback which showed that the simulation session improved their understanding and management of epicardial pacemakers.

Ultrasound simulation model for different disciplines

This combined physical model and virtual reality system is used for training of ultrasound-related examinations and interventions. It provides residents and practising physicians an opportunity to acquire and improve their sonography-related skills on a variety of virtual patients. Ultrasonic task of the following can be performed and trained: basics skills, RUSH (Rapid Ultrasound in Shock), eFAST (extended Focused Assessment with Sonography for Trauma), abdominal, neck, lung, transesophageal echocardiography (TEE), COVID-19, basic gynaecology, trimester screening, fetal echo, fetal neurosonsography; interventional ultrasounds include thoracentesis and central venous catheter. Pericardiocentesis is coming soon.
Simulation cases from one's own patients can be scanned and uploaded as well for augmented training scenarios.

Virtual and physical training in neurosurgery

The "Mycro" device is a small but highly functional training system for microvascular anastomosis and microsuture. On a pad, a membrane can be spanned and put under pressure. Life-like microvessels with dissectable adventitia can be perfused to simulate blood flow. End-to-end and side-to-side anastomoses can be practised; free online courses are available as well. Due to the pressure, the sutures can be checked for leaking. The kit comes with membranes and vessels and instruments like forceps, needle holders, micro scissors and surgical loupes with led light. Further applications for many other surgical disciplines are planned.

Virtual simulator for bronchoscopy training

Virtual training simulator for arthroscopic interventions

Virtual training simulator for endovascular interventions

The training simulator platform is for the practising of endovascular procedures and techniques. The technology enables realistic visualization of the anatomy and instrument activity. It is combined with a haptic system for visual and tactile feedback, which realistically mimics the look and feel of actual endovascular interventions.
Different modules provide basic and advanced cases. The system features more than 35 different endovascular procedures and over 220 patient scenarios. Additional software can create patient-specific 3D digital models based on a patient’s CT, so the actual intervention can be planned and rehearsed prior to surgery on the patient.

Wound care models

The wound care range can be used to train in identifying and assessing different types of wounds. The lifelike material of the models is flexible and allows for the washing and cleaning of wounds as well as the application of dressings and negative pressure wound therapy devices. The Seymour model is based on a 74-year-old patient with pressure injuries, blisters, a pressure injury with undermining, tunnelling, subcutaneous fat, and eschar, and a deep injury with exposed bone, undermining, tunnelling, subcutaneous fat, and eschar. The Wilma's foot is based on an 80-year-old patient with 20 (pathologic) conditions such as ulcers (stage 1 to 4), callus, amputation, corns, blisters, hammer toe, and various sores. The Stan model has a large Stage 4 sacral compression injury with eschar, subcutaneous fat, undermining, tunnelling and exposed bone with osteomyelitis and a Stage 3 compression injury with subcutaneous fat and granulation tissue. The Annie leg model shows various (pathological) conditions such as necrotic toes, fissures, (neuropathic) ulcers and amputated toes and has an ultrasonic Doppler module.
